10 C 6-301.14: A sign or poster that notifies FOOD EMPLOYEES to wash their hands shall be provided at all HANDWASHING SINKS used by FOOD EMPLOYEES and shall be clearly visible to FOOD EMPLOYEES.
There are no hand washing signs at the bathroom hand washing sinks. Hand washing signs were provided and posted.
11 PF 3-201.11 (C): (C) PACKAGED FOOD shall be labeled as specified in LAW, including 21 CFR 101 FOOD Labeling, 9 CFR 317 Labeling, Marking Devices, and Containers, and 9 CFR 381 Subpart N Labeling and Containers, and as specified under §§ 3-202.17 and 3-202.18.
Canned goods with no label were stocked on shelves for distribution. These cans were removed from the shelves and moved to an area for discard. COS

Observed shell eggs stored above ready to eat foods in the stainless cooler. These eggs were moved to the bottom shelf. COS

There was no hot water available in the women's bathroom. Repairs were made and hot water is now available. COS
51 C 5-203.13(A): (A) At least 1 service sink or 1 curbed cleaning facility equipped with a floor drain shall be provided and conveniently located for the cleaning of mops or similar wet floor cleaning tools and for the disposal of mop water and similar liquid waste.
There is no service sink in the facility. Install a service sink.
Inspection Comments   THIS INSPECTION IS FUNCTIONING AS THE PRE-OPENING INSPECTION AND ORIGINAL INSPECTION. KCHD WAS NOTIFIED IN EARLY DECEMBER THAT THIS ESTABLISHMENT MOVED TO THIS LOCATION SEVERAL MONTHS EARLIER AND HAS BEEN OPERATING SINCE THIS TIME. THIS ESTABLISHMENT PREVIOUSLY OPERATED AS LIBERATION WORSHIP CENTER AT 164 E OAK ST, KANKAKEE. NO PROCEDURES HAVE CHANGED. MOST EQUIPMENT WAS MOVED FROM THE PREVIOUS LOCATION.

AS THERE WAS NO CONSTRUCTION, NO REFERRAL FORM HAS BEEN COMPLETED. HOWEVER, COPIES OF FIRE AND CODE INSPECTIONS WILL BE SUPPLIED TO KCHD.

THE REFRIGERATOR IN THE COLD STORAGE ROOM IS KEEPING FOOD TEMPERATURES AT 44 DEGREES. HOWEVER, THERE ARE NO TCS FOODS STORED IN THIS REFRIGERATOR.
