THIS INSPECTION IS FUNCTIONING AS THE PRE-OPENING INSPECTION AND ORIGINAL INSPECTION. KCHD WAS NOTIFIED IN EARLY DECEMBER THAT THIS ESTABLISHMENT MOVED TO THIS LOCATION SEVERAL MONTHS EARLIER AND HAS BEEN OPERATING SINCE THIS TIME. THIS ESTABLISHMENT PREVIOUSLY OPERATED AS LIBERATION WORSHIP CENTER AT 164 E OAK ST, KANKAKEE. NO PROCEDURES HAVE CHANGED. MOST EQUIPMENT WAS MOVED FROM THE PREVIOUS LOCATION.
AS THERE WAS NO CONSTRUCTION, NO REFERRAL FORM HAS BEEN COMPLETED. HOWEVER, COPIES OF FIRE AND CODE INSPECTIONS WILL BE SUPPLIED TO KCHD. THE REFRIGERATOR IN THE COLD STORAGE ROOM IS KEEPING FOOD TEMPERATURES AT 44 DEGREES. HOWEVER, THERE ARE NO TCS FOODS STORED IN THIS REFRIGERATOR. |
